School and Sports Physicals 2010-2011
For established patients entering elementary, high school or college.Type of Visit:
Wellness Visit. Please make appointments as soon as possible. As the school
year approaches, it may take up to two weeks for a scheduled wellness visit.
What is included:
Thorough physical exam and completion of any required physical forms including
immunization history. The required physical forms are available at the school
or can be downloaded and printed from here.
Vaccinations:
Required vaccinations will be administered during this visit, providing
the necessary immunization history is on file. Please note that there is
a separate charge for each vaccination. Please bring a copy of all immunization
records.
Insurances Accepted:
See accepted insurance plans.
Note about insurance:
Not all insurance plans cover wellness visits. Not all insurance plans cover
vaccinations.
Please check with your individual insurance carrier for details of your
personal coverage.
Cash paying patients:
Wellness physicals cost approximately $100 and vaccinations are a separate
charge.
The price of each vaccination is different.
There will be a $10 administration fee for duplication of lost physical forms.
